Participation Requirements - All athletes must have a current physical from their doctor prior to any sports participation. This physical MUST be on the OHSAA Physical Form only. This form can be found on the Athletic Website under the "Forms & Links" tab. All athletes must be registered and have completed all required forms on the Final Forms system for participation. Athletes must also sign-up for the sport they wish to tryout. The link to Final Forms can be found in the "Forms & Links" section of the Athletic Website.
